This article introduces a new hybrid intake procedure developed for posttraumatic stress disorder (PTSD) screening, which combines an automated textual assessment of respondents’ self-narratives and item-based measures that are administered consequently. Text mining technique and item response modeling were used to analyze long constructed response (i.e., self-narratives) and responses to standardized questionnaires (i.e., multiple choices), respectively. The whole procedure is combined in a Bayesian framework where the textual assessment functions as prior information for the estimation of the PTSD latent trait. (...) The purpose of this study is twofold: first, to investigate whether the combination model of textual analysis and item-based scaling could enhance the classification accuracy of PTSD, and second, to examine whether the standard error of estimates could be reduced through the use of the narrative as a sort of routing test. With the sample at hand, the combination model resulted in a reduction in the misclassification rate, as well as a decrease of standard error of latent trait estimation. These findings highlight the benefits of combining textual assessment and item-based measures in a psychiatric screening process. We conclude that the hybrid test design is a promising approach to increase test efficiency and is expected to be applicable in a broader scope of educational and psychological measurement in the future. Dissension is rife not only between people but also within them, for each of us is subject to a contrariety of desires, beliefs, motivations, aspirations. What image are we to form of ourselves that might best enable us to accept the reality of discord, or achieve the ideal of harmony? Greek philosophers offer us a variety of pictures and structures intended to capture the actual and (...) the possible either within a reason that fails to be resolute, or within a split soul that houses a play of forces. Reflection upon them alerts us to the elusiveness at once of mental reality, and of the understanding by which we hope to capture and transform it. Rudolf Carnap is widely regarded as one of the most important philosophers of the twentieth century. Born in Germany and later a US citizen, he was a founder of the philosophical movement known as Logical Empiricism. He was strongly influenced by a number of different philosophical traditions, and also by the German Youth Movement, the First World War, and radical socialism. The Human A Priori is a collection of essays by A. W. Moore, one of them previously unpublished and the rest all revised. These essays are all concerned, more or less directly, with something ineliminably anthropocentric in our systematic pursuit of a priori sense-making. Part I deals with the nature, scope, and limits of a priori sense-making in general. Parts II, III, and IV deal with what are often thought to be the three great exemplars of the systematic pursuit of (...) such sense-making: philosophy in the case of Part II, ethics in the case of Part III, and mathematics in the case of Part IV. Much of the attention throughout is devoted to the work of other philosophers: Kant and Wittgenstein feature prominently, and five of the essays take the form of reviews or critical notices of recent work in philosophy. But the interest in never purely exegetical. One of the lessons that emerges from the essays, either in opposition to the views of these other philosophers or by invocation of their views, is that we humans achieve nothing of real significance in philosophy, ethics, or mathematics except from a human point of view, and hence that all three of these pursuits can be said to betoken what may reasonably be called 'the human a priori'. In this bold and innovative new work, A.W. Moore poses the question of whether it is possible for ethical thinking to be grounded in pure reason. In order to understand and answer this question, he takes a refreshing and challenging look at Kant’s moral and religious philosophy. Identifying three Kantian Themes – morality, freedom and religion – and presenting variations on each of these themes in turn, Moore concedes that there are difficulties with the Kantian view that morality can be (...) governed by ‘pure’ reason. He does however defend a closely related view involving a notion of reason as socially and culturally conditioned. In the course of doing this, Moore considers in detail, ideas at the heart of Kant’s thought, such as the categorical imperative, free will, evil, hope, eternal life and God. He also makes creative use of the ideas in contemporary philosophy, both within the analytic tradition and outside it, such as ‘thick’ ethical concepts, forms of life and ‘becoming those that we are’. Throughout the book, a guiding precept is that to be rational is to make sense, and that nothing is of greater value to use than making sense. (shrink)

A.W. Moore presents eighteen of his philosophical essays, written since 1986, on representing how things are. He sketches out the nature, scope, and limits of representation through language, and pays particular attention to linguistic representation, states of knowledge, the character of what is represented, and objective facts or truths.

The paper builds on the postulate of “myths we live by,” which shape our imaginative life (and hence our social expectations), but which are also open to reflective study and reinvention. It applies this principle, in particular, to the concepts of love and vulnerability. We are accustomed to think of the condition of vulnerability in an objectifying and distancing way, as something that affects the bearers of specific (disadvantaged) social identities. Against this picture, which can serve as a pretext for (...) paternalist and controlling attitudes to the groups in question – notably to women – Anderson urges us to reimagine our vulnerability as a condition not merely of exposure to violence but of openness to mutual affection, love, and friendship. Hegel’s celebrated image of the owl of Minerva, which takes wing only with the coming of dusk, suggests an association of (philosophical) wisdom with negativity – with the experience of death or loss. Anderson, by contrast, proposes an alternative and more hopeful image of the dawn of enlightenment, in the guise of new ethical dispositions shaped by an emancipatory (and so, no longer an oppressive or sexist) conception of our capacity for love. Her main interlocutors or influences in this piece are Judith Butler (who contributes the idea of a “relational ontology”), Michèle Le Doeuff, and Mary Midgley. (shrink)

Background Medical ethics deals with the ethical obligations of doctors to their patients, colleagues and society. The annual reports of Sri Lanka Medical Council indicate that the number of complaints against doctors has increased over the years. We aimed to assess the level of knowledge, attitude and practice regarding medical ethics among doctors in three teaching hospitals in Sri Lanka. Methods A hospital-based cross-sectional study was conducted among doctors using a pre-tested self-administered, anonymous questionnaire. Chi Squared test, and ANOVA test (...) were used to identify the significance of association between level of knowledge and selected factors. Results Most doctors had a poor level of knowledge on medical ethics, with postgraduate trainees showing significantly higher level of knowledge. The average knowledge on medical ethics among doctors was significantly different between the three hospitals. Over 95% had a favourable attitude towards gaining knowledge and advocated the need for training. The majority indicated awareness of unethical practices. 24.6% of respondents stated that they get a chaperone ‘sometimes’ during patient examination while 3.5% never do. The majority responded that they never accept gifts from pharmaceutical companies in recognition of their prescribing pattern. 12–41% of doctors participated in the study acknowledged that they ‘sometime’ engaged in unethical practices related to prescribing drugs, accepting gifts from pharmaceutical companies and when obtaining leave. Conclusion Most doctors had a poor level of knowledge of medical ethics. Postgraduate trainees had a higher level of knowledge than other doctors. The majority showed a favourable attitude towards gaining knowledge and the need of training. Regular in-service training on medical ethics for doctors would help to improve their knowledge on medical ethics, as well as attitudes and ethical conduct. (shrink)

Although a common clinical phenomenon, delusions are difficult to explain and have a problematic conceptual status. Advances in understanding delusions have come from studies which involve detailed investigation of particular types of delusion. Some of this work is summarised, with the Capgras and Cotard delusions as specific examples. These are used to high-highlight questions for which there is the potential for fruitful dialogue with philosophers. Such questions include the criteria for deciding that a statement represents a belief, the extent to (...) which we integrate our beliefs into a coherent web, and the nature and limits of human rationality. In Thucydides' account of Melesander's expedition with six ships to Caria and Lycia there appears to me to be a difficulty which is universally ignored by commentators and fudged by translators. Thucydides describes the purpose of the expedition.

I am enormously grateful to everyone who has contributed to this double issue of Philosophical Topics, to Manuel Dries and Joseph Schear for conceiving the issue and initiating the process of inviting contributions, and to Ed Minar and Jack Lyons, former editor and current editor of the journal respectively, for their excellent work in bringing the issue into existence. Each contribution displays a level of engagement with my book1 that would have been gratifying even if the contribution had been confined (...) exclusively to the chapter that is its immediate focus. What I have found most gratifying, however, is the sensitivity that the contributors have shown to the broader project of the book and to the various connections and patterns that I try to trace across chapters. I thank them warmly for this.In the preface to my book I comment on the fact that I am a philosophical generalist, and I go on to apologize to “all those whose expertise I may have propagated without acknowledgement, or mangled, or worst of all ignored” (MST, xix). How, in pursuit of ontological neutrality, should one talk about values? I propose to say: there are values. Those three words do nothing to define within what kind of conception of a world values are at home.1 I take it that the ‘realist’ must have more to say about values and their world. I recognize that an ‘anti-realist’ may prefer to talk of value-terms ; I ask him to wait and see whether taking the linguistic turn is the only way (...) to put values in their place. (shrink)

This book brings together essays on themes of human rights and legal history, reflecting the long and distinguished career as academic writer and human rights activist of Brian Simpson. Written by colleagues and friends in the United States and Britain, the essays are intended to reflect Simpson's own legal interests. The collection opens with biography of Simpson's academic life which notes his major contribution to legal thought, and closes with an account of his career in the United States and a (...) bibliography of his writings. As a tribute to Simpson's varied interests in the law, the collection is grouped around themes in human rights, legal philosophy, and legal history. The human rights papers are concerned with the history of the right of individual petition to the European Court of Human Rights, and recent successes in which Brian Simpson played a part; the evolution of a transnational common law of human rights; the United Nations Convention on the Rights of the Child and the interpretation of the provisions on identity in France and England; the suspension of human rights which would have occurred, had the emergency War Zone Courts scheme been brought into effect during wartime; historical resistance to colonial laws in Papua New Guinea; and the ratio decidendi of the story of the Prodigal Son. Historical themes are found in essays concerned with three nineteenth-century Lord Chancellors; in two essays relating to the fate of the civil jury on either side of the Atlantic which provide a fascinating comparison; in the 'battle of the books' which led to changes in eighteenth-century copyright law; and judicial rivalry between King's Bench and Common Pleas in the early modern period. This book is concerned with the history of metaphysics since Descartes. Taking as its definition of metaphysics 'the most general attempt to make sense of things', it charts the evolution of this enterprise through various competing conceptions of its possibility, scope, and limits. The book is divided into three parts, dealing respectively with the early modern period, the late modern period in the analytic tradition, and the late modern period in non-analytic traditions. In its unusually wide range, A. W. Moore's (...) study refutes the tired old cliché that there is some unbridgeable gulf between analytic philosophy and philosophy of other kinds. It also advances its own distinctive and compelling conception of what metaphysics is and why it matters. Moore explores how metaphysics can help us to cope with continually changing demands on our humanity by making sense of things in ways that are radically new. (shrink)

The United Kingdom Central Council for Nursing, Midwifery and Health Visitors (UKCC) document, Exercising Accountability, states that the role of patient's advocate is an essential aspect of good professional nursing practice (1). The author examines the case for and against the nurse being the best person to act as advocate, and critically evaluates the criteria of advocacy. The problematic moral issues arising are discussed, and a case made for negotiation between the members of the multidisciplinary team and the patient/client (or (...) a significant person to the patient) in order to promote the well-being of the patient and to minimise suffering. She concludes that the health care professional's (including the nurse's) role is to help people to assert control over the factors which affect their lives, that is empowerment, rather than advocacy. Different questions generate different forms of practical reasoning. A contextually unrestricted ‘What shall I do?’ is too open to focus reflection. More determinately, an agent may ask, ‘Shall I do X, or Y?’ To answer that, he may need to weigh things up—as fits the derivation of ‘deliberation’ fromlibra(Latin for ‘scales’). Ubiquitous and indispensable though this is, I mention it only to salute it in passing. Or he may ask how to achieve a proposed end: if his end is to (...) do X, he may ask ‘How shall I do X?’ Or he may ask how to apply a universal rule or particular maxim. Aristotle supplies examples inDe Motu Animalium(7.701a7 ff.), whose wording I freely adapt to my own purposes:A1 reasons to a necessary means to achieving an end:I will make a cloak.To make a cloak I must do A.So, I will do A. A. W. Moore argues in this bold and unusual book that it is possible to think about the world from no point of view. His argument involves discussion of a very wide range of fundamental philosophical issues, including the nature of persons, the subject-matter of mathematics, realism and anti-realism, value, the inexpressible, and God. The result is a powerful critique of our own finitude. 'imaginative, original, and ambitious' Robert Brandom, Times Literary Supplement.

John Cottingham identifies “the grand traditional project of synoptic ethics” as an attempt to define the essential features of a good human life within a rational understanding of the world, and of man’s place within it. That the project now seems dated he explains in two ways. First, he notes the recent specialization and professionalization of philosophy, its preference of technical topics to grand questions. Second, he adduces a skepticism that doubts the objectivity, and a liberalism that accepts a plurality, (...) of values. The upshot of both trends is a “committee ethics” that takes on “the task of articulating the structure of an optimal political system in which maximum scope is given for the pursuit of whatever values are in fact chosen by the citizens” —a grandish issue, surely, but neither a personal nor a metaphysical one. Cottingham’s main aim is to cast further doubt on the practicability of the traditional project so long as it is pursued in the traditional “ratiocentric” style. What that is he conveys through discussions of Plato, Aristotle, and Descartes. (shrink)
